Department Overview
Family Medicine at Gabriel Park is a community clinic located in SW Portland. We serve our patients in varying stages of life and throughout their lives. Our services include general family medicine, transgender care, maternity care (including deliveries), pediatric care, behavioral health, sports medicine, travel immunizations, acupuncture, a retail pharmacy, x-ray, minor procedures, and health maintenance with focused preventive care. We are a high-volume clinic that sees up to 250 patients and receives 400-600 phone calls daily. We have 39 providers plus 60 staff members. Our clinic guides itself though our vibe: Compassionate, Committed, and Collaborative.
The Behavioral Health Consultant (BHC) position is a behavioral health provider who operates within the primary care team at OHSU Family Medicine at Gabriel Park providing point of care behavioral interventions to the primary care population.
The BHC role is to detect and address a broad spectrum of behavioral health needs in the primary care patient population with the aim of early identification, quick resolution, long-term prevention and wellness. The BHC functions as a core member of the primary care team.
This position involves a high degree of clinical complexity and advanced level skill in delivering evidence-based interventions targeted to improve wellbeing for a variety of patients across the lifespan. The BHC seeks to support a smooth coordination of care with community partners and completes short term case management for patients during their episodes of care.
OHSU Family Medicine at Gabriel Park serves patients with substance use disorders, embraces harm reduction, and provides pharmacotherapy for opioid use disorder. The BHC will work with patients with substance use disorders and assist in the coordination of their care.
The BHC will work within established practices and procedures with opportunities to participate in the planning, design and implementation of workflows. The BHC performs duties with minimal supervision. Work is reviewed for adherence to procedures, regulations, standards and for overall effectiveness.
Function/Duties of PositionProvide same day consultation to providers and evidenced based interventions with patients be available to care team members for point of care Warm Hand Offs and proactively identify patients with unmet behavioral needs.
Provide brief supportive counseling or other behavioral treatments to assist in their care. Support care team and patients in achieving care plan goals. Assist with case management of high-utilizing and complex patients.
Provide clinical services, supportive counseling, relapse prevention, and care coordination for patients with substance use disorders and on medication assisted treatment.
Participate in education and training of care team members and medical residents in behavioral medicine competencies
Lead the enhancement of integrated behavioral health services within the primary care clinic including workflow, program devolvement, and provider training
- Offer consultation and intervention for effective relationships between patient and clinic or between patient and care team member consistently utilize professional communication and conflict resolution skills to help resolve conflicts that might arise in the course of medical care.
Required Qualifications
A Master's Social Work (MSW) required.
Certificate of Clinical Social Work Associate (CSWA) or Licensed Clinical Social Worker (LCSW) required for employees who practice clinical social work.
- Experience in behavioral health counseling and brief short-term therapy strategies and case management is required.
- Supervised clinical experience
Job Related Knowledge, Skills and Abilities (Competencies):
- Thorough knowledge and understanding of patient-centered medical home principles, motivational interviewing skills and evidence-based short-term therapy strategies.
- Ability to utilize assessment information and collateral information to conceptualize patient psychopathology.
- Knowledge of the principles and practices of behavioral health consultation, including assessment, diagnosis, treatment planning, cognitive-behavioral counseling strategies and their application to population-based care for preventive and chronic care issues.
- Comprehensive understanding of the interrelationship between medical and psychological systems and their impact on health.
- Skill and expertise in conducting group and individual psycho-educational classes and brief, short-term therapy strategies that will help strengthen the patient's overall physical and mental health.
- Incorporation of self-management, home-based practice in care planning.
- Experience in documenting clinical activities in a comprehensive, clear, concise fashion on a same day basis.
- Skill in efficiently organizing time so that unscheduled, same day services are routinely available to care team members.
- The ability to exercise sound judgment in appraising situations and making decisions regarding complex physical and behavioral health treatment issues in consultation with the primary care teams.
- Skill in developing effective working relationships with peers, supervisors and professionals in related disciplines.
- Ability to manage complex personal interactions.
- Demonstrated ability to act with minimal supervision on personal initiative using sound clinical judgment.
- Strong written and verbal communication skills.
- Ability to maintain confidentiality in regard to patient care information and chart contents as use of patient record and privacy of individuals must be protected to the fullest.
- Ability to maintain documentation in compliance with State, Federal, Professional and Ethical guidelines.
Preferred Qualifications
- At least two years of supervised clinical experience
- Health care experience preferred.
- Formal training in Primary Care Behavioral Health.
- Experience as a Behavioral Health Consultant in a Primary Care setting; certification in addictions (CADC) is preferred.
- 3 years experience in similar position is preferred.

Hours: Mon-Fri, flexible hours
The work is performed in a medical setting and involves normal working hours with occasional alternative hours for special programs or outreach. The position involves interactions with people who have a wide variety of physical and behavioral health problems. Some travel may be required.
