About this role

One of out clients is actively seeking a LBSW for Community Case Management and Support Services to help assist the newly housed population in Nashville, TN.

CLIENT HOURS
* Primary work hours are 8:00 AM- 4:30 PM M-F
* However, this may extend into additional hours for cold weather shelter
* Approximate length of engagement will be 6 months

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITYY
* Providing direct case management for individuals and families, including intake, assessment, service planning, and follow up.
* Conducting outreach in the community, shelters, and identified program sites to engage clients and support access to services.
* Coordinating referrals to housing, behavioral health, social services, and other MRRF supported resources.
* Maintaining accurate documentation, case notes, and reporting according to program and compliance standards.
* Participating in multidisciplinary team meetings and collaborating with OHS, partner agencies, and community providers.
* Supporting clients with crisis intervention, problem solving, benefits navigation, and linkage to long term supports.
* Working on site when required, and being flexible in responding to community based needs across multiple locations

CLIENT'S REQUIRED SKILLS AND EXPERIENCE
* Bachelors Degree minimum
* Licensed Baccalaureate Social Worker (LBSW) - Tennessee Verified
* Experience working with the Unhoused Population
