As a Social Worker in our facility, you'll play a key role in helping residents and their families navigate care transitions, emotional challenges, and support services. This is a unique opportunity to make a meaningful difference in the lives of both short-term rehab patients and those requiring long-term support.
 
What You’ll Do:

Provide emotional support and counseling to residents and families
Coordinate care plans in collaboration with our interdisciplinary team
Advocate for resident needs and preferences
Ensure documentation complies with regulatory standards
