Summary:
Northern Light Health Mercy Cancer Care Program is seeking an experienced licensed clinical social worker to join a dynamic, patient-centric multidisciplinary team. The Licensed Clinical Social Worker provides care to cancer patients, families, and their caregivers throughout the active treatment, surveillance, survivorship, and end of life continuum. Provides assessment, social services, and counseling to patients and their families in relation to social, psychological, financial, and family situations to promote quality of life and optimal patient outcomes. Opportunities for professional growth community outreach, and program development. Must have strong clinical, interpersonal, and organizational skills. The Cancer Program is accredited through the American College Commission on Cancer.
Responsibilities:
Acts collaboratively with the Cancer Care Center multidisciplinary team and Mercy Financial Counseling on psycho-social issues to meet the needs of the patient/family. Including participation in new patient teaching day, tumor boards, and the Survivorship program Builds relationships with patients, families, and healthcare teams to coordinate and advocate on behalf of patients' goals of care Assesses adaptation of families to patients' illnesses, provides crisis intervention, short-term therapy as needed, and ensures maximum participation of patients/families in the continuum of care process Assess and respond to the practical concerns for patients accessing treatment, including financial assistance, transportation, insurance, employment, food & housing insecurity. Address obstacles to accessing services for patients in coordination with referral sources and other staff Demonstrates commitment to cultural sensitivity and cultural competence with diverse populations Initiates and/or participates in interdisciplinary collaboration to implement psychosocial treatment: Ability to initiate and facilitate interdisciplinary meetings to discuss and/or implement psychosocial treatment plan Communicates effectively with patients, families, care manager and other members of the health care team Collaborates with other community agencies to promote health, provide education and develop better programs in the community and facilitate referrals to meet patient needs.
