Key Responsibilities
- Conduct comprehensive assessments and evaluations of patients impacted by cancer, utilizing tools such as the Diagnostic and Statistical Manual of Mental Disorders to create personalized treatment plans that address the unique cultural and individual needs of each client.
- Employ evidence-based interventions to support patients throughout their treatment process, actively monitoring progress and adapting the care plan as necessary. This involves regular check-ins and updates to ensure the effectiveness of therapeutic strategies.
- Maintain meticulous treatment records and clinical documentation, utilizing state-of-the-art technology to ensure compliance with regulatory standards and internal procedures.
- Collaborate with patients, families, and healthcare providers to develop comprehensive treatment objectives focused on patient-centered care, ensuring that all parties are aligned with the goals of therapy.
- Coordinate referrals and liaise effectively with external treatment providers to ensure continuity of care for your clients, fostering strong collaborative relationships within the healthcare community.
- Provide mentorship and educational opportunities for students, interns, and volunteers, integrating them into ongoing clinical processes while enhancing their understanding of mental health practices.
- Work under the guidance of clinical supervisors to engage in peer review activities, contribute to team development initiatives, and participate in chart audits aimed at improving clinical outcomes and team performance.
Patient Population / Client Focus
This role primarily serves individuals and families impacted by cancer—patients who often face a multitude of challenges, including emotional distress, behavioral health issues, and social adjustment difficulties. This focus requires you to be sensitive to the complex realities these patients face, employing empathy and understanding to provide tailored support in a compassionate manner.
Qualifications
- A Master's degree in Social Work from an accredited institution.
- Active and current State License as a Licensed Independent Clinical Social Worker (LICSW).
Preferred Experience
- Experience working in a healthcare setting focusing on oncology or chronic illness support.
- Strong background in evidence-based practices and individualized patient care strategies.
- Prior experience providing supervision and mentorship to social work students or trainees is advantageous.
Compensation and Benefits
Community Uplifters LLC offers a competitive salary range of USD 90,000 – 105,000 annually, commensurate with experience.
