- Perinatal Social Worker (Hybrid: Remote/In-person/In-home)
Description
The purpose of this position is to provide support services to women during the perinatal period, which encompasses pregnancy, childbirth, and the postpartum period. This role involves social determinants of health (SDoH) and behavioral health screening assessments and resource coordination to address the complex psychological and social needs that can arise during this time. A Perinatal Social Worker (SW) specializes in providing behavioral health screening assessments and facilitating coordination of care to women during the perinatal period. LCSWs and MSWs will be on-staff to support Bridge Prenatal (BP) members and serve as “resource companions” for engaged Medi-Cal enhanced care management patients. The SW establishes contact with women upon referral to Bridge Prenatal to conduct comprehensive psychosocial assessments in the perinatal period, identifying mental health conditions, social challenges (SDoH) and risk factors.
Requirements
- Master of Social Work (MSW) degree or Licensed Clinical Social Worker (LCSW) licensure.
- Experience in mental health counseling, with a focus on perinatal and women’s health issues (perinatal loss/bereavement, infertility, prenatal, postpartum).
- Strong clinical skills in assessment, intervention, and treatment planning.
- Excellent communication and interpersonal skills, with the ability to work empathetically with diverse populations.
- Knowledge of community resources and systems relevant to perinatal care.
- Computer skills (experience with Google, Excel, Virtual meeting platforms) and experience with electronic health record documentation is strongly encouraged.
- Experience with developing individualized care plans to address identified social drivers of health barriers and behavioral health related needs.
- Familiarity with behavioral health questionnaires including EPDS, GAD-7, and PHQ-9.
- Bilingual candidates (English/Spanish) are preferred.
