Description

Sierra Medical Center seeks an RN Case Manager or Social Worker to join our Oncology Department. In this role, you will coordinate comprehensive care for oncology patients, guiding them through diagnosis, treatment, and survivorship. You’ll conduct needs assessments, develop individualized care plans, arrange services, and provide psychosocial support to patients and families. Collaborating closely with physicians, nurses, and research staff, you will help manage transitions of care, support participation in clinical trials, ensure accurate documentation in the EHR, and promote compassionate, patient-centered care in a team-oriented environment.
Coordinate oncology patient care across inpatient, outpatient, and community settings
Conduct comprehensive assessments and develop individualized care plans
Provide psychosocial support, education, and resource navigation for patients and families
Collaborate with physicians, nurses, and interdisciplinary teams on treatment goals
Support patient participation and consent processes for oncology clinical trials
Facilitate safe transitions of care and discharge planning
Advocate for patient needs with payers and community agencies
Document assessments, plans, and outcomes in the electronic health record (EHR)
Monitor patient progress and adjust care plans as needed
Participate in quality improvement and staff education initiatives
Oncology nursing or oncology social work
Clinical case management
Care coordination and discharge planning
Psychosocial assessment and counseling
Knowledge of oncology treatments and side-effect management
Clinical trial coordination support
